Support at Home assessment: how to make sure your needs are heard
Published 29 September 2026. An aged care assessment should reflect what daily life is really like, including the tasks that are difficult, the help you rely on and what could happen without support.

Prepare, explain and check
Apply
Contact My Aged Care online or on 1800 200 422. Have your Medicare details ready.
Get ready
List health conditions, medicines, current support, safety concerns and tasks that have become harder.
Be specific
Describe what happens on difficult days and what you can only do with help, prompting or supervision.
Cover the whole picture
Discuss health, home tasks, mobility, falls, memory, thinking, current supports, goals and preferences.
Read the outcome
Check your Notice of Decision, support plan, approved services, reasons and review rights.
Request a review
If important information is missing or the decision seems wrong, follow the internal review instructions in your letter.
Do not minimise the help you need
People often describe what they can do on a good day. For an accurate assessment, explain the usual level of difficulty, how often help is needed, what takes longer, what causes pain or fatigue and any risks such as falls, missed medicines or carer strain.
You may have a family member, friend, carer or registered supporter present. Reports from your GP or other health professionals can also help explain your needs.
After the assessment
You will receive a Notice of Decision and, if eligible, a support plan. If you disagree with the outcome, you can request an internal review. Keep copies of reports and information that support what you need to remain safe and independent.
