Assistive technology and home modifications: what Support at Home can fund
Published 29 September 2026. Assistive Technology and Home Modifications, usually shortened to AT-HM, provides separate funding for approved equipment and home changes that help an older person live safely and independently.

AT-HM at a glance
Assessment
An aged care assessor considers your needs and may approve assistive technology, home modifications, or both.
Assistive technology
Examples include walking aids, wheelchairs, commodes, shower chairs and other approved equipment.
Home modifications
Examples include handrails, ramps, stair lifts, bathroom changes and wider doorways.
Funding tiers
Low: under $500. Medium: up to $2,000. High: up to $15,000, with some assistive technology available above this when assessed.
Funding is separate from ongoing services
AT-HM funding is separate from a participant's ongoing quarterly Support at Home budget. It can cover an approved item or modification and, where required, professional assessment, prescription, installation and other wraparound services.
Funding is generally available for 12 months. High-tier home modification funding can be extended to 24 months when progress is demonstrated, while high-tier home modifications are generally capped at $15,000 per person over their lifetime.
How to access AT-HM
- Explain the safety or independence problem during an aged care assessment.
- Ask whether an AT-HM funding tier should be included in your support plan.
- Read your Notice of Decision and support plan carefully.
- Work with your provider and any required health professional to select an approved item or modification.
Only items on the official AT-HM list that meet assessed needs can be funded. Ordinary household expenses, workplace equipment and items more appropriately funded elsewhere are excluded.
