Aged care pathways at a glance
Published 29 September 2026. A My Aged Care assessment is usually the starting point for government-funded aged care. The right pathway depends on your needs, eligibility, circumstances and local availability.

Common aged care pathways
CHSP
Entry-level help with everyday tasks, transport, social support and some home services.
Support at Home
Coordinated ongoing or short-term support for people with more complex or intensive needs.
Transition care
Short-term support after a hospital stay to help recovery and independence.
Residential respite
A short stay in an aged care home, including to give a carer a break.
Permanent residential care
Accommodation with ongoing personal and nursing care when living independently is no longer possible.
Short-term Support at Home
AT-HM, restorative care and end-of-life pathways may be approved separately according to assessed need.

Contact My Aged Care on 1800 200 422 or apply online. If you are in hospital, the discharge team can help connect you with appropriate services.
CHSP remains the main entry-level home support program and has been extended to 30 June 2029. Support at Home is designed for more complex or intensive needs and includes ongoing services plus short-term pathways.
Choosing the right pathway
Your assessor considers your health, safety, independence, current help and goals. Access depends on the assessment outcome, eligibility and availability in your area. If needs change, contact My Aged Care or your provider to discuss whether reassessment is appropriate.
